A woman with advanced nonsmall cell lung cancer was started on pembrolizumab monotherapy and developed severe liver injury 9 months later. Interviews with her and her family revealed that she had purchased oral fenbendazole based on social media reports suggesting that it was effective against her cancer, and she had been self-administering the drug. After discontinuation of the fenbendazole, her liver function returned to normal.
The active ingredient in fenbendazole is an organic compound called benzimidazole. It has broad antiparasitic activity in animals and is a potent microtubule inhibitor. It also has antitumor activity and inhibits RAS-related signaling pathways in cells with a KRAS mutation. Several studies have shown that fenbendazole is also able to reduce cellular proliferation. It is a member of the benzimidazole class of drugs, which includes metronidazole and albendazole.
